- Application is complete (no blank spaces)
- Affidavit of Character is complete and signed by Affiant
- Your signatures- "Oath of Office"
- Print your name under your signature
- First timers- Education Certificate
- Non-Citizens- Declaration of Domicile
- Your signature only- Bond of Notary Public
